• Certification Details
• Residency Declaration Form as required for compact license.
• ACLS - Advanced Cardiac Life Support (AHA only)
• BLS - Basic Life Support (AHA only) as required
• PALS - Pediatric Advanced Life Support as required
• NRP - Neonatal Resuscitation Procedure as required
• TNCC - Trauma Nursing Core Course as required
• Tech Cert Verification - National
• HIPAA Safety Manual Acknowledgement required
• State License Verification required
• NURSYS Verification (RN, LPN/LVN, AP) as required
• NIH Stroke Certification required for all RNs and LPN/LVNs
• CPI - Crisis Prevention Institute required as applicable
• Medication Test/Pharmacology Competency Exam required
• Competency Exam required
• Core Competencies required for Subcontractors

        • Flu Seasonal Vaccination/Documentation - 2024-25 required
        • Confidentiality Agreement - Must complete prior to start
        • Proper respirator fit test will be conducted onsite at orientation

          • Government issued photo ID required. Must be carried on person during hours of service.
          • Please require if nurse is working on a compact license.
          • As required. AHA certifications only.
          • Not required for MLT or SPT.
