Job Description:
We are seeking a Physical Therapist to join our team at St.
Mary's Home Health, a part of LHC Group, to provide high-quality
care to patients in their homes.
Job Description:
As a Physical Therapist, you will be responsible for assessing and
evaluating patient care needs related to functional
status/mobility, endurance/stamina, pain and other medical
conditions. You will work with the patient and their family to
develop a treatment plan, perform interventions aimed at improving
and enhancing the patient's well-being, and evaluate the patient's
progress.
Responsibilities:
--- Evaluate the patient's functional status and physical therapy
needs
--- Assist the physician in the development of the therapy plan of
care
--- Observe and report the patient's response to treatment and any
changes in their condition to the supervising nurse and/or the
physician
--- Treat patients to relieve pain, develop and restore function,
and/or maintain maximum functioning using acceptable standards of
practice
--- Instruct and inform participating members of the healthcare
team, the patient, and the family/caregivers regarding the plan of
care, functional limitations, and progress toward goals
Requirements:
--- Current Physical Therapy licensure in the state of
employment
--- Current CPR certification
--- Must have a current driver's license and vehicle insurance, and
access to a dependable vehicle
